Membership forms and affiliation

Membership and affiliation forms are now available.
This year there are five different affiliation forms available, as the fees vary according to age. Please make sure you complete the right one! The required fee is at the bottom of the form.
The different categories are:
- 11 and under on 1st September 2012
- 14 and under on 1st September 2012 NOT playing in the senior club
- 14 and under on 1st September 2012 playing in the senior club
- 18 and under on 1st September 2012
- Over 18 on 1st September 2012
Please can members complete a form and return it to Trudie with the correct affiliation fee, or with a note confirming that you have transferred the correct fee electronically as soon as possible.
Players who will be taking part in either the Swindon League pre-season tournament on Saturday 1st September or the Regional League 2/3 tournament on Sunday 2nd September must be affiliated to take part.
